Does a photo this small actually need retouching?

It depends on what needs fixing. The ERAS file is compressed to a small resolution, so the smallest details simply don't register — a single hair flyaway isn't worth touching, because it disappears at that size. But the bigger improvements do carry through. Noticeably clearer skin, neater hair, and cleaner clothing change the overall tone, contrast, and impression of the image — and that reads even at the small ERAS size, and again when your photo is enlarged on a screen as a selection committee discusses candidates. The rule of thumb: skip the micro-details, and retouch where there's a real, visible improvement to make.

Worth it vs. skip it

Worth retouching: uneven or blemished skin, shine, stray strands that break the hairline, wrinkled or distracting clothing, glasses glare, teeth and eye brightness.

Not worth it: a stray flyaway or two, a freckle, anything so fine it vanishes once the file is sized for ERAS.

Why does my formatted photo look blurry full-screen?

That's normal — it doesn't mean the photo is low quality or that the retouching is soft. The ERAS file is formatted to the AAMC's required small resolution: 375 × 525 px, under 150 KB. When you open a file built for that small size and blow it up to fill a full screen, it looks soft, because you're viewing it far larger than it was ever meant to be seen. At the size programs actually display it, it's sharp. Every order also includes the high-resolution master for anything that needs full sharpness at large size — LinkedIn, faculty pages, or print.
