Internal medicine is the largest applicant pool in the Match. PDs see thousands of photos a season. The photo that lands well is warm, confident, and entirely unremarkable in its execution.
The IM aesthetic
If there is a default ERAS photo, IM is it. The applicant pool is enormous, the programs are diverse, and the photo that reviews well across community, university, and academic IM programs is consistent: a navy or charcoal blazer over a white or light blue shirt, a relaxed half-smile, eyes direct to camera, no fuss. Boring is not the enemy here — distinctive in a bad way is the enemy.
Wardrobe
- Navy blazer over a crisp white or light blue button-down. The single most common choice in the IM applicant pool, for good reason. Photographs cleanly against the light gray seamless, reads professional, no patterns to pixelate.
- Charcoal suit jacket if you want slightly more formal. Works just as well.
- Solid-color sweater or fine-knit cardigan if a blazer feels off. Stick to deep solid colors — navy, burgundy, dark green.
- Tie optional. Most applicants skip it. If you wear one, keep it solid or with micro-texture only.
Expression
IM programs value warmth + competence. A natural half-smile lands better than either a wide grin or a closed-mouth professional smile. The eyes do most of the work — relaxed at the corners, direct to camera, present rather than tense.
Common IM mistakes
- Trying too hard to look serious. IM PDs aren't selecting on sternness. The "doctor face" reads as anxious in a photo. Conversational expression wins.
- Stripes and small patterns. Houndstooth, fine stripes, gingham, repeating textures — all pixelate at 150 KB. Solid colors only.
- Stale photo from M3 year. Many IM applicants reuse a photo from clerkship orientation. If it's more than a year old, refresh.
Plus tier for IM applicants
IM applicants apply broadly — 30+ programs is common, 60+ is not unusual. Many also apply to fellowship a year or two later.
