Pediatrics is the one specialty where the photo can — and probably should — read warmer than the residency default. Peds PDs are selecting for clinicians who can connect with families on the worst day of their lives. The photo gets to telegraph that.
The peds aesthetic
If surgery is the most conservative residency photo and IM is the default, peds has the most permission to be warm. That doesn't mean casual — the AAMC spec still applies, and a pediatric attending in clinic still wears a structured top. But the expression, posture, and palette can lean toward open and approachable in ways that would feel off in other specialties.
Wardrobe
- Navy or charcoal blazer over a button-down or simple top. The standard works fine.
- Softer palette is OK. A muted dusty blue, deep teal, or warm burgundy blouse / shirt reads warmer than pure navy without losing professionalism. This is one of the few specialties where this works.
- Sweater or cardigan over a collared shirt is more common in peds than in any other specialty. If it's how you actually dress in clinic, it's fine in the photo.
- Solid colors only. The pattern rule still applies — patterns pixelate at 150 KB regardless of how cheerful they are.
Expression
A real, warm smile is welcome here. Not a wide grin — but a present, eyes-engaged half-smile that suggests I am someone you would want talking to your scared three-year-old. Eyes warm at the corners. Shoulders relaxed. The expression that works for peds is closer to "favorite aunt or uncle" than "doctor."
Common peds mistakes
- Going too soft. Pediatrics applicants sometimes overcorrect into casual. The photo is still a residency application. Structured silhouette, eyes present, posture squared up.
- Forced "warm doctor" smile. If the smile doesn't reach your eyes, it doesn't land. The photographer's job is to direct your attention onto something that actually makes you smile — most photographers we work with have a routine for this.
- Wearing scrubs to look "real" or "approachable." No.
